Join our Lifestyle Medicine Small Medical Group, led by Jade Spellmeier APRN, FNP-C, DipACLM. In these 6 sessions you will learn how lifestyle changes can help prevent, manage, and potentially reverse chronic diseases. These sessions will be held in a small group setting of peers with similar health concerns as we work together towards the common goal of health improvement.
What is Lifestyle Medicine?
It is an evidence-based medical specialty that uses therapeutic lifestyle interventions as a modality to treat chronic conditions. Jade is a Lifestyle Medicine Certified Clinician and is trained to apply evidence-based, whole-person, prescriptive lifestyle change to treat and, when used intensively, often reverse chronic conditions.
Who should sign up?
Patients with chronic diseases such as Type 2 Diabetes, pre-diabetes, hypertension, high cholesterol, obesity, and metabolic syndrome will likely benefit from this program.
